Overview
Long-term colorectal cancer incidence refers to the rate at which new colorectal cancer cases occur in a defined population followed over an extended period, a measure used to track disease burden, evaluate prevention and screening strategies, and understand the natural history of the disease. Colorectal cancer typically develops through a multi-step progression from precursor lesions, including conventional adenomas and the serrated pathway, to invasive carcinoma, a sequence that creates opportunities for early detection and interruption. Long-term incidence is shaped by the effectiveness and uptake of screening programmes, the biology of precursor lesions, and the quality of detection, with research examining factors such as taller haustral folds in the proximal colon that may contribute to interval cancers arising between screening episodes. Screening modalities, including faecal immunochemical testing and colonoscopy, influence incidence by identifying and removing precancerous lesions, and population- and company-based programmes demonstrate how organised screening affects detection rates and compliance, while psychological responses to screening can affect participation. Molecular tools such as microRNA quantification are studied for diagnostic screening of colon cancer, and chemopreventive approaches, including the role of nonsteroidal anti-inflammatory drugs, have been investigated for their potential to reduce incidence. Such research informs efforts to reduce the long-term occurrence of colorectal cancer and the burden it imposes.
