Overview

Heart failure is a clinical syndrome in which the heart cannot pump blood at a rate sufficient to meet the metabolic demands of the body, or can do so only at elevated filling pressures, resulting in fatigue, dyspnoea and fluid retention. It may arise from impaired systolic contraction, with reduced ejection fraction, or from impaired diastolic relaxation with preserved ejection fraction, and commonly follows coronary artery disease, hypertension, valvular and rheumatic disease, cardiomyopathy or congenital lesions. Pathophysiology involves adverse cardiac remodelling, neurohormonal activation and cardiorenal interaction, which drive progression and underlie acute decompensation. Heart failure is increasingly prevalent and carries substantial morbidity and mortality, and its management combines treatment of the underlying cause, risk-factor control, pharmacological and device therapy, and emerging molecular approaches. Diagnosis relies on clinical assessment, biomarkers, imaging and functional evaluation. Research collected under this term reflects these dimensions: gene therapy for heart failure, heart failure in primary care, cardiorenal signalling pathways, rheumatic heart disease, assessment of cardiac function and sleep-disordered breathing, pericarditis after heart transplantation, lung-damage mechanisms in COVID-19, Ebstein's anomaly, vital-organ biomarker studies, microRNA strategies in ischaemic heart disease, and coronary chronic-total-occlusion intervention. The peer-reviewed literature in this area spans the causes, pathophysiology, cardiorenal interactions, diagnosis, and pharmacological, device-based and molecular management of impaired cardiac pump function.
